Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and referral sources about at-home and alternate-site infusion services. You will assess patient suitability, coordinate with hospital teams, and support timely discharge with a strong focus on relationship-building and sales outcomes.

The role requires RN or Pharmacist licensure with 2+ years in healthcare, plus strong communication and PC skills. Travel to partnerships and facilities is expected.

Qualifications

• Experience establishing and maintaining relationships with individuals at all levels of the organization and with vendors.
• Experience applying knowledge of standard practices for all services offered and relevant standards (ACHC, URAC).
• Experience providing customer service to internal and external customers, meeting quality standards and evaluating satisfaction.
• Basic PC skills: internet, email, MS Word/Excel/PowerPoint.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Serves as key point of contact to educate and assist referral sources.
• Interacts with area service providers to sell infusion services for discharging patients.
• Assesss patients to ensure suitability for alternative site care and timely discharge.
• Coordinate education for patients and caregivers to facilitate therapy independence.
• Communicate with medical staff and family to implement effective discharge plans.
• Collaborate with Account Manager to create and execute growth plans.
