Job summary

Option Care Health is seeking a Clinical Transition Specialist to drive sales with discharge planning expertise. The role educates patients and facility staff on services, coordinates with internal and external partners, and builds strong referral relationships to grow patient starts.

Applicants must hold RN or Pharmacist licensure and bring at least two years in healthcare. Travel to care facilities and hospitals is required as part of this role.

Qualifications

• Licensed RN or licensed Pharmacist in the state of practice.
• Minimum 2 years in healthcare and related service delivery.
• Experience building relationships with clinicians, facilities and referral sources.

Responsibilities

• Maintain and grow relationships with referrals sources to increase sales.
• Educate patients, families and facility staff about services and products.
• Coordinate discharge planning and ensure seamless patient transitions to home care.
• Collaborate with Account Managers to execute area business and growth plans.
• Assist with patient education and documentation during pre-admission processes.
