Job summary

Option Care Health is seeking a Clinical Transition Specialist, an experienced sales professional who educates patients, their families and facility staff about services and products delivered at an alternative site.

You will build and nurture referral relationships, collaborate with intake, case managers and discharge planners to ensure timely, high‑quality transitions to home or ATS, and work with Account Managers to grow referrals and hit sales goals.

Qualifications

• Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice and at least 2 years of experience in the healthcare industry.

Responsibilities

• Maintain and grow relationships with referral sources to increase patient starts.
• Educate patients, families, and staff about services and products.
• Collaborate with hospital staff to assess patients for home/alternate site care.
• Coordinate care transitions with internal and external partners.
• Partner with Account Manager to execute area growth plans.
• Ensure accurate pre-admission information and signoffs.
