Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and care teams on home infusion services. You will build strong relationships with referral sources and hospital staff to drive patient starts and ensure smooth transitions to home or alternate sites.

Ideal candidates hold an RN or Pharmacist license and have 2+ years in healthcare, with strong communication and presentation skills and proficiency in MS Office.

Qualifications

• Licensed RN or Pharmacist in the practicing state is required.
• Healthcare industry experience is required (2+ years).
• Understanding of ACHC and URAC standards is preferred.
• Strong customer service and relationship-building skills.
• Proficient with MS Office applications and basic tech.

Responsibilities

• Maintains and grows relationships with referral sources to drive sales and patient starts.
• Educates patients, families, and facility staff about services and products.
• Provides hands-on education to patients and caregivers for timely discharge.
• Coordinates with internal and external partners to ensure seamless transitions.
• Collaborates with Account Manager to execute area growth plans.
• Collects prerequisite information and secures approvals/signoffs.
• Maintains confidentiality and complies with privacy regulations.
• Arranges medical supplies and equipment as needed.
• Represents Option Care to referral sources and providers.
