Job summary

Whitney Rehab Care Center in Hamden, CT seeks an experienced RN Infection Preventionist to lead infection prevention and control across the facility. You will conduct surveillance, audits, and staff training to safeguard residents and staff.

The role includes outbreak investigations, antibiotic stewardship, regulatory compliance with CMS/CT-DPH, and reporting to leadership. Strong organizational and communication skills are essential.

Qualifications

• Current Registered Nurse (RN) license in Connecticut.
• Minimum of 2 years of nursing experience in a long-term care or skilled nursing facility.
• Knowledge of infection prevention standards specific to long-term care (CDC, CMS, and state regulations).
• Strong organizational, communication, and staff education skills.
• Ability to conduct audits, analyze data, and implement improvement strategies.

Responsibilities

• Develop, implement, and maintain the facility’s Infection Prevention and Control Program in accordance with CDC, CMS, and state regulations for long-term care.
• Conduct ongoing surveillance for healthcare-associated infections and communicable diseases among residents and staff.
• Lead outbreak investigations and implement containment and prevention strategies (e.g., influenza, COVID-19, etc).
• Educate and train nursing staff, CNAs, and ancillary departments on infection prevention practices to ensure infection prevention standards are maintained facility-wide.
• Ensure compliance with regulatory and survey requirements (CMS and CT-DPH)
• Maintain antibiotic stewardship practices
• Monitor adherence to infection control policies
• Participate in Quality Assurance and Performance Improvement (QAPI) meetings and present infection control data and trends.
• Maintain accurate documentation and prepare reports for leadership and regulatory agencies as required.
• Serve as the facility subject matter expert for infection prevention and communicable disease management
• Current Registered Nurse (RN) license in the state of Connecticut.
