Job summary

Whitney Rehabilitation Care Center in Hamden, CT seeks a Registered Nurse Infection Preventionist to lead the facility’s infection prevention and control program, conduct surveillance, and ensure compliance with CDC, CMS, and CT-DPH standards.

You will educate staff, perform outbreak investigations, support QA/QI activities, and report trends to leadership, while maintaining accurate documentation.

Qualifications

• Current CT RN license required.
• Minimum 2 years nursing in long-term care or skilled nursing facility.
• Knowledge of infection prevention standards for long-term care (CDC, CMS, CT regulations).
• Strong organizational and communication skills; staff education ability.
• Ability to conduct audits, analyze data, and implement improvements.

Responsibilities

• Develop, implement, and maintain the facility's Infection Prevention and Control Program per CDC, CMS, and CT regulations for long-term care.
• Conduct ongoing surveillance for healthcare-associated infections among residents and staff.
• Lead outbreak investigations and implement containment strategies.
• Educate nursing staff, CNAs, and ancillary departments on infection prevention practices.
• Ensure compliance with CMS and CT-DPH regulatory surveys.
• Maintain antibiotic stewardship practices.
• Monitor adherence to infection control policies.
• Participate in QAPI meetings and present infection control data and trends.
• Prepare documentation and reports for leadership and regulatory agencies.
• Serve as the facility subject matter expert for infection prevention and communicable disease management.
