Job summary

Signature HealthCARE in Kentucky is seeking an experienced RN for our Liberty Care and Rehabilitation Center. You will provide direct nursing care, supervise nursing staff, manage medications, and ensure resident safety and dignity in a team-driven environment.

The role requires an active RN license, 1+ year of clinical nursing experience in long-term care, and strong observation, documentation, and communication skills. CPR/BLS is preferred, and familiarity with EHR systems is valued.

Qualifications

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience, long-term care preferred.
• Supervisory or charge nurse experience preferred.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Excellent verbal and written communication.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides on shift per policy.
• Monitor residents’ status and report changes to physicians/teams.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
