Signature HealthCARE is seeking an experienced RN to join our extended care team in Kentucky. You will provide medical care, assist with ADLs, and supervise nursing staff to ensure safety and quality of life for residents.

The role emphasizes coordinated care plans, accurate documentation, and regulatory compliance, with opportunities for professional growth and involvement in quality initiatives.

Qualifications

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.
• Strong communication and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and report changes to the physician.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
