Job summary

Carilion Clinic in Roanoke, VA is seeking an RN I for Admission and Discharge coordination across patient care. You will coordinate admission, transfer, and discharge processes, using clinical judgment and collaboration to ensure safe and efficient transitions.

This role requires a Virginia RN license, BLS-HCP and ACLS, and at least one year of acute care experience. You will work with case management and social work to facilitate throughput while delivering compassionate, patient-centered care.

Qualifications

• Graduate of a State Board of Nursing approved program.
• Minimum of 1 year of acute care experience.
• Current RN license in the Commonwealth of Virginia; BLS-HCP and ACLS.
• Strong communication and customer service skills; basic computer proficiency.

Responsibilities

• Coordinate patient admission, transfer and discharge processes.
• Provide nursing coordination and individualized care for patients of all ages.
• Collaborate with case management and social work to facilitate throughput.
