Carilion Clinic in Roanoke, VA is seeking an RN I for Admission and Discharge coordination across patient care. You will coordinate admission, transfer, and discharge processes, using clinical judgment and collaboration to ensure safe and efficient transitions.
This role requires a Virginia RN license, BLS-HCP and ACLS, and at least one year of acute care experience. You will work with case management and social work to facilitate throughput while delivering compassionate, patient-centered care.
How You’ll Help Transform Healthcare: The RN I Admission and Discharge coordinates patient care and supports admission, transfer, and discharge processes, utilizing clinical judgment and collaboration to ensure safe and efficient transitions. The RN I Admission/Discharge As a part of the healthcare team, assumes primary accountability for nursing coordination and provision of individualized for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborated approach to patient care. Assists in facilitation of the admission, transfer, and discharge processes. Assesses, plans, implements and evaluates patient care in order optimum patient outcomes for patients of all ages as appropriate to service area. Determines and preforms within clinical, leadership and/or education pathways to support (direct/indirect) quality patient care service and performance improvement through anticipating and effetely functioning in crisis/emergency situations based on sound judgment, escalation, and use of advanced skills. Manages resources, establishes prioritizes, and modifies environment to meet patient care needs. Collaborates with case management and social work to facilitate throughput. Assists in the coordinates the admission, transfer, and discharge process across disciplines and service lines. Actively contributes to the overall positive patient experience through the use of excellent customer service skills. Utilizes bed tracking technology and collaborates with leadership and transfer center to facilitate throughput.
Compensation Range: $33.52 - $50.28 Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
