Home Health & Hospice Care in Merrimack, NH is seeking a Team Leader RN to bridge administrative leadership and hands-on clinical operations, balancing scheduling, triage, and field visits in a hybrid role. Reporting to the Department Team Manager, you’ll coordinate care, manage field staff, and participate in weekend rotations to ensure high-quality, cost-effective community health services.

The ideal candidate holds an RN license in NH with 3–5 years of clinical experience, including home care

Qualifications

• Associates or better in Nursing (NH RN license preferred).
• CPR Certification required.
• Drivers License.

Responsibilities

• Coordinate & Schedule Care: match patients with field staff based on acuity and competency, optimizing scheduling for clinical efficiency.
• Triage & Communicate: field incoming calls from clients, families, and vendors and process orders from physician offices.
• Support Field & Admin Operations: perform field visits, participate in weekend/holiday rotations, and assist management with quality initiatives.
• Lead Hospice Initiatives: manage patient pharmacy needs, oversee DME ordering, and lead IDT meetings when the manager is absent.
