Job summary

Home Health & Hospice Care in Merrimack, NH is hiring a Registered Nurse for hospice home care. You will deliver skilled, compassionate patient care, complete assessments, and develop individualized care plans with families.

This role requires an Associate Degree in Nursing (BSN preferred), at least one year of acute care experience, CPR/BLS certification, and a willingness to travel within the assigned territory; weekend and holiday rotation apply. Sign-on bonus up to $2,500.

Qualifications

• Associates degree in Nursing (BSN preferred).
• 1 year of acute care experience (end of life, hospice, palliative, or oncology preferred).
• CPR/BLS certification; valid driver’s license.

Responsibilities

• Complete initial and ongoing home assessments.
• Develop individualized care plans with measurable goals.
• Coordinate interdisciplinary care with physicians and the care team.
• Teach and counsel patients and families on self‑care and disease management.
• Prepare timely clinical notes and maintain communication with the care team.
• Travel within assigned territory and assist outside the usual area.
