Job summary

Amedisys in Clermont, FL is seeking a Part-time RN Field Nurse for Hospice. The role covers weekend shifts (Saturday-Sunday, 8:00 AM-8:00 PM) with travel within Lake County and requires an active RN license and CPR certification.

You will assess patients, develop care plans with the care team, document progress, and educate families on care techniques. Onboarding includes 7–9 weeks of orientation and ongoing professional growth in a supportive team.

Qualifications

• Current and valid RN license in the state(s) where you will work.
• Minimum 1 year of strong clinical RN experience in an acute healthcare setting.
• Current CPR certification.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Salary Range:

$39- 42 hourly, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
