Job summary

Compassionate Care Hospice, an Amedisys Company, is hiring an RN Field Case Manager in Clermont, FL. The role requires weekend travel within Lake County, with Saturday and Sunday shifts 8:00 AM - 8:00 PM.

You will assess patients, develop care plans with the team, and educate families while coordinating with nurses and managers. Training includes 7-9 weeks orientation tailored to experience.

Qualifications

• Current RN license in the state(s) where you will work
• Minimum 1 year of strong clinical RN experience in an acute healthcare setting
• Current CPR certification
• Hospice or home health experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Salary Range

$39-42 hourly, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.
