Job summary

Amedisys in Sweetwater, TX, is seeking an RN Case Manager for a full-time daytime role. The opportunity includes a $7,500 sign-on bonus and competitive pay ranging from $70,000 to $75,000 annually, with care delivered in patients' homes across designated regions.

The role involves patient assessments, care-plan development with the care team, referrals as needed, documentation, and family instruction, with on-call duties and a focus on compassionate home-based care.

Qualifications

• One year of nursing experience.
• Current RN license.
• Current CPR certification.
• Valid driver's license with reliable transportation and insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
