Amedisys Hospice of Brevard in Melbourne, FL is seeking an experienced RN Admissions Nurse to join our team. The role requires coordinating admissions, assessing patients' needs, and setting up the initial plan of care with the physician and interdisciplinary team.

The position offers a Monday-Friday, 10:00 AM–7:00 PM schedule with no weekends or on-call, travel within the territory, competitive pay, full benefits, and opportunities for growth as you support patients and families in our hospice

Qualifications

• Current RN license for Florida.
• Minimum 2+ years of clinical RN experience in acute care.

Responsibilities

• Coordinates admissions of hospice patients by assessing initial physical, psychosocial and spiritual needs.
• Establishes the initial plan of care with the attending physician and interdisciplinary team.
• Provides skilled interventions for symptom control and patient comfort under physician direction.
• Acts as liaison between disciplines and facilities during admission.
• Maintains individualized patient care progress records with timely documentation.
• Communicates changes in condition to the clinical manager/physician and hospice team.

This position is based in Melbourne, FL and requires daily travel within assigned territory. Flex Schedule: Monday-Friday, 10:00 AM - 7:00 PM, No Weekends or On-call.

Salary Range

$85,000-90,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
• Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Minimum 2+ years of clinical RN experience in acute healthcare setting(s)
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
