AHCA Level 2 fingerprint screening is required. Reporting to the Manager of Hospice Coordination, the position is based at the Pasco Home Health Office in Land O Lakes, FL.

• Facilitates referrals to hospice and assists in developing relationships with community business partners.
• Performs timely, individualized, accurate assessments for patients referred to Gulfside Healthcare Services for Hospice services as they relate to Local Coverage Determinations.
