Job summary

Gulfside Healthcare Services is seeking an RN Liaison/Admission at the Pasco Home Health Office in Land O Lakes, FL. The role focuses on facilitating hospice referrals, building community partnerships, and delivering timely, accurate patient assessments in line with Local Coverage Determinations.

In addition to required RN licensure, the position may involve AHCA Level 2 background screening. This on-site role reports to the Manager of Hospice Coordination and supports Gulfside’s hospice

Also, this position is responsible for performing timely, individualized, accurate assessments for patients referred to Gulfside Healthcare Service for Hospice services as they relate to the Local Coverage Determinations.
