Job summary

Constellation Health Services is seeking a Hospice RN Clinical Liaison to serve the Canton, MA area. You will plan, organize and direct home care services, with emphasis on community health education and building relationships with hospitals and facilities.

The role expands business opportunities by networking with social workers, physicians and key decision makers, managing patient care, and coordinating with a clinical team. Travel and on-call duties are included.

Qualifications

• Registered nurse with licensure and CPR certification.
• Bachelor’s degree in nursing; 1–2 years acute care experience preferred.
• Experience in home health care, marketing or sales is a plus.
• Must have a valid driver’s license with insured vehicle.
• Excellent planning, organization and presentation skills.

Responsibilities

• Assess patient and family to determine home care needs and plan care.
• Re-evaluate nursing needs and adjust care plans as status changes.
• Coordinate patient care as Case Manager when assigned.
• Communicate with physicians and community partners; update notes every 60 days.
• Develop sales/marketing strategies and build relationships with hospitals and facilities.
• On-call duties and ensure equipment and services are arranged.
