Job summary

Constellation Health Services in Cincinnati, OH seeks an experienced Registered Nurse to serve as an RN Clinical Liaison for the Cincinnati territory. The role focuses on planning, directing home care services and expanding partnerships with hospitals, skilled nursing and physician offices to grow hospice and home health referrals.

The ideal candidate has acute care nursing experience, strong communication and case management skills, and is willing to develop community relations and marketing

Qualifications

• Graduate of an accredited school of nursing.
• Current state licensure and CPR certification.
• Bachelor’s degree in nursing or equivalent preferred for home health coordination.

Responsibilities

• Perform initial patient assessment and develop plan of care.
• Coordinate patient care and act as Case Manager as assigned.
• Communicate with physicians and family members about patient needs every 60 days or as required.
• Coordinate equipment and services; supervise home health aides every two weeks.
• Engage in on-call duties as defined by policy.
• Develop and implement sales/marketing plans to expand hospice and home health referrals.
