KeystoneCare in Pennsylvania is seeking an energetic Clinical Hospice Liaison to advocate for patients and families, partnering with hospital teams to streamline care transitions to hospice, palliative care, and home health.

You will perform clinical assessments, coordinate with case managers, discharge planners, and social workers, and educate families on options while ensuring timely access and compliance with eligibility criteria.

Qualifications

• Must hold PA RN license and be in good standing.
• Minimum 3 years as clinical care coordinator, liaison, or case manager.
• Strong time management and communication abilities.
• Clinical knowledge of diseases common to home health and hospice.
• Valid driver''s license and reliable vehicle.

Responsabilités

• Serve as the primary clinical liaison between hospital partners and KeystoneCare.
• Build and maintain relationships with case managers, discharge planners, physicians, and social workers.
• Conduct clinical evaluations to determine hospice eligibility.
• Collaborate to ensure smooth transitions from hospital to home or appropriate care setting.
• Educate patients and families on hospice, palliative care, and options.
• Review medical records for clinical appropriateness and eligibility.
• Maintain on-site presence at facilities for real-time coordination.
• Facilitate referrals and strengthen provider relationships.
