Job summary

Carolinas Medical Center is seeking a part-time Registered Nurse for the 34141 WI Hospice Divisional - Nursing: SCT (Supportive Care Team). The role involves coordinating patient care planning, scheduling interdisciplinary team meetings, and guiding staff with new material and care standards.

Qualifications

Bachelor's Degree in Nursing required.
License to practice as a Registered Nurse.
Experience in home health or hospice settings preferred.

Responsibilities

Coordinate patient care planning and outcomes with nursing visits and revisits.
Schedule interdisciplinary team meetings and educate staff on changes and new material.
Review quality and operational data to improve clinical outcomes and patient finances.
Assess patient data to establish and adjust the plan of care per physician orders.
Provide direct patient care and adjust nursing processes to meet patient needs.
Document visits, care plans, and orders accurately and promptly.
Assist in discharge planning and ensure continuity of care after discharge.
Participate in quality improvement activities and process enhancements.
