Horizon Home Care & Hospice in Portland, OR is seeking an experienced RN to join our Hospice Inpatient Units for evening shifts. You will assess patients and families, coordinate care, and support end-of-life decisions with compassion and professionalism.

You will perform admission evaluations, document clearly, and collaborate with the interdisciplinary team to deliver high-quality, patient-centered care in a supportive, not-for-profit environment.

Responsibilities

Perform referral evaluations and admission assessments for hospice patients and families.
Conduct comprehensive physical, psychosocial, and spiritual assessments, using cues to obtain complete information.
Communicate patient and family expectations of care and learning needs to the health care team with proper documentation.
Ensure all documentation is clear, legible, concise, timely, and meets department standards.
Provide Case Management as part of the IDG to an assigned caseload.
Approach patients and families with care, concern and compassion.

In Horizon's Hospice Inpatient Units, you will play an important role in supporting and educating patients and their families as they embark on the end-of-life journey. We are seeking experienced nurses to join our Hospice Inpatient team.

Perform referral evaluations / admission assessments of patients and families seeking Hospice services.
Performs physical, psychosocial and spiritual assessments utilizing non-verbal cues and probes or modifies questions to obtain more comprehensive assessments
Communicates patient's and family's expectations of care, goals and comprehensive learning needs to all members of the health care team via appropriate documentation too
All documentation is clear, legible, concise, accurate, timely, and meets department standards
Provide Case Management functions, as part of the IDG, to an assigned case load.
Provide services at quality levels consistent with professional standards of practice Approach patients and families with care, concern and compassion.
