Job summary

The Care Team Hospice in Lisle, IL seeks an experienced Clinical Manager (RN) to lead and support our clinical team, driving high-quality, compassionate end-of-life care. You will partner with branch leadership to oversee daily operations and ensure compliant documentation and patient care coordination.

You will mentor field clinicians, review care plans, and monitor performance metrics to maintain survey readiness and continuous improvement.

Qualifications

• Active RN license required.
• Minimum of two years of RN experience with at least one year in hospice, home health, or related setting.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong leadership, clinical assessment, communication, and organizational skills.
• Ability to coach teams and manage multiple priorities in a fast-paced environment.
• Valid driver's license, reliable transportation, and current automobile insurance.

Responsibilities

• Lead and support daily clinical operations while ensuring hospice regulations and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, and performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and team collaboration.
• Monitor quality metrics, conduct chart audits, and support survey readiness and improvement initiatives.
• Partner with leadership, physicians, referral sources, and operations to promote clinical excellence.
