Job summary

The Care Team Hospice in Lisle, IL seeks a Clinical Manager (RN) to lead and support our clinical team at the Winfield Hospice site. This role focuses on developing clinicians, ensuring quality patient care, and shaping the growth of our hospice program.

As Clinical Manager, you will partner with branch leadership to oversee daily clinical operations, mentor staff, ensure compliance with hospice regulations, and collaborate with physicians, referral sources, patients, and families to deliver

Qualifications

• Active RN license required.
• Minimum of two years of RN experience, including at least one year in hospice or home health leadership.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Lead and support daily clinical operations while ensuring compliance with hospice regulations, company policies, and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
• Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
• Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient and family experiences.
