Compassionate Care Hospice in The Villages, FL is seeking an experienced RN Case Manager to coordinate patient care within our hospice program. The role involves patient assessments, care planning, referrals, and collaboration with the care team; daily travel around Sumter County is required.

• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

This position is based in The Villages, FL and requires daily travel around Sumter County. Schedule: Monday-Friday, 8:00 AM - 5:00 PM.

Salary Range

$83,000–$86,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

• Other duties as assigned.

Qualifications

Required:

• Current RN license in the state(s) where you will work
• Minimum 1 year of clinical RN experience in an acute healthcare setting (Required)
• Current CPR certification

Preferred:

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
