Compassionate Care Hospice, an Amedisys Company, is seeking an RN Case Manager for The Villages, FL. The role involves patient assessments, care planning, and coordination with the care team in a Monday–Friday schedule, with travel across Sumter County.

Qualifications

• Current RN license in the state(s) where you will work.
• Minimum 1 year of clinical RN experience in an acute healthcare setting.
• Current CPR certification.
• Hospice or home health experience preferred.

• Comfortable with daily travel between facilities.
• Proficient in electronic medical record (EMR) systems.

Responsibilities

• Performs patient assessments with the care team to plan care.
• Makes referrals to hospice disciplines or documents rationale for not referring.
• Maintains timely, accurate patient progress records.
• Educates patients and families on care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities as assigned.

This position is based in The Villages, FL and requires daily travel around Sumter County. Schedule: Monday-Friday, 8:00 AM - 5:00 PM.

Salary Range:

$83,000–$86,000 annually, plus bonus opportunities.

Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
