Job Profile Summary

The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit.

Qualifications
  • Current registered nurse (RN) license in the state where the employee will be working.
  • Active BLS certification from the American Heart Association or Red Cross.
  • Minimum one (1) year of nursing experience; hospice or hospital experience preferred.
  • Previous experience with an EMR/EHR system.
  • Valid driver’s license and automobile insurance per company policy; reliable transportation to meet visit schedule.
  • Ability to use equipment with visual and auditory mechanisms.
  • Effective written and verbal communication in English.
  • Flexible schedule to meet the needs of Hospice House patients.
  • Ability to manage emotional stress of working with and caring for terminally ill patients and their families.
  • Physical access to patient rooms and other areas of the Hospice House, hospital, assisted living facility, and home environments.
  • Capability to perform essential functions including lifting patients/equipment, bending, pushing/pulling, kneeling, and other physical tasks as required.
  • Completion of Alzheimer’s Disease and Related Dementias Training (PACE certification) through the Florida Department of Elder Affairs, where applicable.
  • Cross‑training to hospice house or field positions at equal licensure/certification and scope, when needed for flexible staffing.
Core Clinical Duties
  • Completes clinical admissions for all company services, including explaining services, addressing questions, identifying and responding to barriers, educating patients/families, obtaining informed consents, initiating Plan of Care, and coordinating skilled nursing interventions.
  • Obtains physician orders and completes documentation per company policy.
  • Acts as company representative at assigned facilities, facilitating referrals, and maintaining strong relationships with case managers, physicians, and other healthcare professionals.
  • Provides care for GIP patients in hospitals, including daily visits, professional management, documentation, and efficient discharge.
Job Responsibilities
  • Provides and manages direct care to patients and families as part of the Interdisciplinary Group, incorporating psychosocial, spiritual, cultural, physical, and biological components.
  • Coordinates the Plan of Care, ensuring individualization and accuracy as patient needs evolve.
  • Educates patients, families, caregivers, and other health professionals about disease process, decline, palliative interventions, caregiving, dying process, and safety practices.
  • Reports changes in patient condition to appropriate IDG members or other health professionals.
  • Participates with the IDG to evaluate hospice referrals/admissions for level of care appropriateness.
  • Attends patient deaths, prepares body for transport, oversees disposal of medications, and provides bereavement interventions in collaboration with psychosocial staff.
  • Presents concise oral and written reports to IDG, encouraging input from all disciplines.
  • Communicates accurately and completely to physicians, staff members, patients, families, and supervisors using positive approaches.
  • Supervises patient care provided by LPNs, hospice aides, and contract nurses.
  • During emergencies (e.g., hurricanes), may be required to report to alternate locations and be available for extended periods.
  • Performs other duties as assigned.
Physical Demands
  • Ability to see, hear, talk, walk, use hands to finger, handle, or feel.
  • Frequently required to stand, sit, reach with hands/arms, lift, bend, and balance.
  • Occasionally required to pull, push, stoop/crouch, kneel, and climb stairs.
Compensation and Benefits

Pay Rate: $47.00 per hour.

Position requires consent to drug and alcohol testing after a conditional offer, and ongoing compliance with the Drug-Free Workplace Policy. All employees performing services for Florida affiliates are subject to the Florida Care Provider Background Screening Clearinghouse for eligibility verification.
