Job summary

Trustbridge Hospice, a member of Empath Health, seeks an experienced RN Hospice Case Manager in Central Palm Beach County. This PRN role runs 8:30 a.m. to 5 p.m. with weekend availability, coordinating care under the physician and IDG.

Florida RN license is required; BSN preferred. A current BLS certification is needed, with hospice/public health/home health nursing experience preferred. EMR skills and reliable transportation are essential to deliver Full Life Care.

Qualifications

• Registered Nurse with Florida license required.
• BSN preferred for the role.
• BLS certification required.
• Experience in hospice, public health, or home health nursing is preferred.

Responsibilities

• Coordinate patient/family care under the direction of the physician and RN.
• Participate in the interdisciplinary group (IDG) to follow an individualized plan of care.
• Utilize monitoring, teaching, and intervention to maximize comfort and quality of life.
