Job summary

Trustbridge Hospice, a member of Empath Health, is seeking a caring RN Hospice Case Manager for our Central Palm Beach County team. The position is PRN, 8:30 a.m.–5 p.m., with weekend availability.

You will coordinate patient and family care under the physician and IDG, ensuring comfort, dignity, and high-quality hospice services while maintaining confidentiality and compassionate support.

Qualifications

• Registered Nurse currently licensed in the State of Florida.
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing, oncology, medical/surgical, geriatric, or Hospice nursing preferred.

Responsibilities

• Coordinate care for patient/family under the direction of the physician and RN.
• Identify patient/family needs and contribute to the individualized plan of care.
• Collaborate with the interdisciplinary group to maximize comfort and quality of life.
• Provide monitoring, intervention and teaching to patients and families.
• Maintain confidentiality and ensure complete satisfaction of patients and families.
• Deliver high level of personalized, sincere service to patients and families.
