Job summary

Trustbridge Hospice, part of Empath Health, is seeking an RN Hospice Case Manager in the Boca area. The role involves coordinating care with the IDG, developing individualized care plans, and delivering education to patients and families.

The position requires Florida RN licensure and BLS. Schedule is Monday–Friday 8:30am–5:00pm with one weekend every 6 weeks, on-site in Palm Beach area, with competitive pay and strong benefits.

Qualifications

• RN licensed in the State of Florida.
• BLS certification required.
• At least 1 year in public health, home nursing, oncology, medical/surgical, geriatric, or hospice nursing preferred.
• Knowledge of Hospice Nursing concepts: case management, crisis intervention, teaching, and communication.

Responsibilities

• Coordinate care under the direction of the primary physician and interdisciplinary team.
• Develop and implement individualized plans of care.
• Provide clinical interventions and education to enhance comfort and quality of life.
• Advocate for patients and families to address needs promptly.
