Job summary

PruittHealth Hospice is seeking a dedicated Registered Nurse Case Manager (RN) to provide comprehensive case management in home or facility settings. You will evaluate patient needs, coordinate multidisciplinary care, monitor progress, and document in a timely and accurate manner in keeping with policy.

Qualifications

• Current, active, and unrestricted Registered Nurse licensure in the state of practice.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
