Job summary

Trustbridge Hospice, a member of Empath Health, is seeking an RN Hospice Case Manager in the Boca area. This role supports patient-centered care with a Monday–Friday schedule and a weekend requirement every six weeks, serving Palm Beach and Broward counties.

You will lead the interdisciplinary team to coordinate comprehensive care, develop individualized plans, monitor patient conditions, and educate families to improve comfort and quality of life in a compassionate, mission-driven environment.

Qualifications

• Registered nurse licensed in Florida.
• 1 year in hospice, home health, or geriatric nursing preferred.
• Current Basic Life Support (BLS) certification.
• Ability to apply hospice case management concepts and crisis intervention.

Responsibilities

• Serve as an advocate for patients and families, ensuring needs are addressed.
• Coordinate care under physician direction with the interdisciplinary group (IDG).
• Develop and implement individualized care plans as part of IDG.
• Monitor patient conditions, provide interventions, and deliver education to enhance comfort and quality of life.
• Address concerns promptly to maintain high satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is currently seeking a caring and motivated RN Hospice Case Manager to join our team in the Boca area. You would work the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement (One weekend every 6 weeks)

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
