Job summary

Suncoast Hospice, part of Empath Health, seeks a compassionate RN Case Manager for the Hospice Float Pool serving Pinellas County. The role supports home- and facility-based hospice patients with a Monday–Friday 8:00 AM–5:00 PM schedule and occasional weekend coverage, including summer and winter holidays.

You will collaborate with diverse clinical teams, perform assessments, develop care plans, and ensure regulatory compliance while promoting Full Life Care for All.

Qualifications

• Graduate of an approved nursing program.
• Florida RN license required.
• Minimum one year of acute-care hospital experience; hospice RN preferred.

Responsibilities

• Manage a caseload of hospice patients throughout Pinellas County.
• Perform nursing assessments and develop individualized care plans.
• Coordinate care with physicians, CNAs, social workers, chaplains, and other interdisciplinary team members.
• Provide symptom management and patient education.
• Support patients and families navigating end-of-life care.
• Review and update patient care plans, physician orders, and clinical documentation to ensure quality care and regulatory compliance.
