Job summary

Silverado Hospice Temecula is seeking a Director of Patient Care Services to lead hospice care delivery, support interdisciplinary teams, and ensure regulatory compliance with passion and clinical excellence. We aim to provide dignified care to patients and families while mentoring staff and ensuring quality outcomes.

The role blends leadership, nursing expertise, and empathy to empower teams, meet budget goals, and uphold the highest standards of patient-centered care across the organization.

Qualifications

• Graduate of an accredited school of nursing with a bachelor’s degree or higher.
• Current, unencumbered RN license in the state of employment.
• Active CPR certification; Hospice and Palliative Nursing certification preferred.
• Either 2 years supervisory/managerial experience in hospice/home health in last 5 years with a BSN or higher, OR 4 years supervisory/managerial experience in hospice/home health in last 5 years.
• EMR experience preferred; WellSky familiarity highly desirable.
• Valid driver’s license, reliable transportation, and willingness to travel.
• Ability to lead bereavement support groups and make sound clinical decisions.

Responsibilities

• Serve as a role model and resource for nursing staff and interdisciplinary team members.
• Support staff in problem-solving and determining appropriate clinical interventions.
• Collaborate with the Administrator on hiring, orientation, retention, and performance management.
• Adhere to budget requirements for labor, pharmacy, DME, and medical supplies.
• Coordinate patient care with RN Case Managers and attending physicians.
• Review and manage patients receiving higher levels of care (respite, inpatient, continuous care).
• Oversee clinical and management responsibilities for assigned teams; liaison between Administrator, clinical staff, patients, families, and community partners.
