Job summary

Cleveland Clinic is seeking an RN Hospice Coordinator at its Main Campus in Cleveland, OH to ensure smooth hospital-to-home transitions and coordinate full continuum of care. You will work within an interdisciplinary team to coordinate services and educate patients and families about hospice benefits.

The role requires BSN preferred, RN licensure, BLS, and 1+ year in clinic and acute/critical care settings; schedule is full-time day shift.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• BSN preferred.
• Current state RN licensure.
• BLS certification required.

Responsibilities

• Coordinate services as part of the interdisciplinary team.
• Process inpatient referrals into hospice and ensure continuity of care.
• Educate potential customers about hospice benefits.
• Explain hospice benefits to patients and families.
• Handle inquiries for new referrals (internal and external).
• Arrange medical supplies and DME for hospital-to-community transition.
• Assess potential patients in line with hospice policy.
• Plan delivery of patient care with other disciplines.
• Assess safety risks, abuse, and neglect; document changes in condition and care effectiveness.
