Job summary

The ideal candidate is a registered nurse with RN licensure, BLS, and hospital or clinic experience, willing to travel to Akron, Medina, and nearby communities while ensuring seamless transitions and quality patient care.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as an RN.
• One year of clinic RN experience; one year in acute med/surg or critical care.

Responsibilities

• Coordinate services with internal and external referral sources.
• Process inpatient referrals into hospice and coordinate discharge planning.
• Educate customers about hospice benefits and coordinate care plans.
• Document changes and plan of care in the EMR; attend interdisciplinary meetings.
