Job summary

UnitedHealth Group is seeking an RN Case Manager to coordinate compassionate hospice care, assess patients, and partner with the interdisciplinary team to develop individualized plans that meet each patient’s needs. You will document care, adjust plans as conditions change, and support families through transitions in care.

This role requires an active RN license, CPR certification, and at least 1 year of nursing experience, plus a valid driver's license and reliable transportation for home

Qualifications

• Current RN license, state(s) licensed.
• Current CPR certification.
• 1+ years of nursing experience.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to plan care.
• Refers to other hospice disciplines or documents rationale for not referring.
• Develops and maintains individualized patient care progress records.
• Records pain and symptom management changes/outcomes.
• Teaches patients, families, and caregivers appropriate care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services.
• Other duties as assigned.
