Job summary

Gentiva Hospice is seeking a Patient Care Manager to oversee clinical operations and coordinate patient care for our home-based hospice program, reporting to the Executive Director. You will lead care delivery, manage visits, triage referrals, and document minutes from IDG meetings.

The ideal candidate is an RN with at least 3 years direct patient care, hospice/home health experience, and strong leadership and regulatory knowledge. BSN preferred; familiarity with EMR and telehealth is a plus.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in hospice/home health.
• Knowledge of Medicare/Medicaid, JCAHO, ACHC regulations.
• Ability to lead a diverse care team and manage regulatory requirements.
• Strong interpersonal, organizational, and leadership skills.
• Proficiency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present patient updates, and document minutes.
• Mentor and train associates to support development, performance, and compliance.
• Ensure care delivery meets federal, state, and local standards and company policies.
• Support performance improvement initiatives and operational efficiencies.
