Rosaires Care is seeking a Hospice Admissions RN to coordinate admissions for patients in their homes. You will conduct assessments, explain services, and start initial care plans, working closely with physicians and families.

The ideal candidate is an RN with hospice admissions experience, strong communication, and solid documentation skills to ensure compliant, timely service initiation and seamless transitions of care.

Qualifications

• RN licensed; hospice admissions experience preferred.
• Strong clinical assessment and patient-centered communication.
• Ability to educate patients and families about hospice benefits and consent.
• Thorough knowledge of documentation and regulatory requirements.
• Excellent collaboration and organizational skills.
