Position: Hospice Admission/Visit Nurse

As a Hospice Admission/Visit Nurse, you will be responsible for ensuring the delivery of quality home hospice services and coordinating care that aligns with the philosophy and standards of hospice nursing. Your duties include:

1. Assessing patient and family/caregiver conditions accurately during admissions and visits, including physiological, psychosocial, and spiritual aspects.
2. Identifying and addressing patient and family educational needs.
3. Providing patient care according to physician orders and agency policies.
4. Completing all documentation thoroughly, accurately, and promptly to reflect the care provided.
5. Transcribing, implementing, and evaluating new physician orders accurately.
6. Collaborating with patients, care providers, families, and agency staff.
7. Communicating patient issues and concerns to the appropriate team members promptly and accurately.

Qualifications:

• Degree from an accredited School of Nursing (BSN preferred)
• Current WI RN license
• 1-2 years of Hospice, Oncology, or related experience
• Experience caring for the terminally ill; knowledge of Hospice Care
• Hospice certification is preferred
• Valid WI Driver's License and reliable, insured vehicle

Position Details: Full-time, 9am-5:30pm, exempt, per visit basis, hospice home care.
