As their trusted nurse, you work with your patient and our interdisciplinary team of therapists, home health aides, and nurses to implement that plan. As a Home Care RN Case Manager, you will:

1. Complete patient plan of care and submit admission paperwork.
2. Perform ongoing assessments of patients, follow physician orders, and revise plans based on assessment findings.
3. Identify patient goals, develop a plan, and implement interventions.
4. Inform physician, clinical manager, and team members of changes in patient status, documented appropriately.
5. Audit clinical records upon recertification and discharge to ensure compliance with regulations and policies.
6. Ensure appropriate discharge planning from the start of care, utilizing a multidisciplinary approach.
7. Coordinate with the Prior Authorization Nurse regarding ongoing service needs, providing accurate assessment data.

• Degree from an accredited school of nursing (BSN preferred)
• Current WI RN license
• 1-2 years of Med/Surg or home care experience
• Valid WI driver's license and reliable, insured vehicle

Part-time position, 32 hours/week, Monday-Friday, first shift. No weekends, no evenings, no on-call. Includes holiday rotation.

Per visit, exempt position with an $8,000 hiring bonus.
