As a Home Care RN Case Manager, you will:

1. Complete patient plan of care and submit admission paperwork.
2. Perform ongoing assessments of patients, follow physician orders, and revise the plan of care based on assessment findings.
3. Identify patient goals, develop a plan, and implement interventions.
4. Inform physicians, clinical managers, and team members of changes in patient status through documentation.
5. Audit clinical records upon recertification and discharge to ensure compliance with regulations and policies.
6. Ensure appropriate discharge planning from the start of care, utilizing a multidisciplinary approach and proper documentation.
7. Coordinate with the Prior Authorization Nurse regarding ongoing service needs, providing accurate assessment data.
