Amedisys is seeking an admissions registered nurse (RN) for full-time daytime shifts in Trevose, PA. You will coordinate hospice admissions, assess patient needs, and collaborate with physicians and the care team to establish a plan of care.

The role emphasizes patient comfort, timely documentation, and effective communication with caregivers. Qualified candidates hold an active RN license in the state, with at least 1 year of RN experience and CPR certification.

• Minimum of 1 year clinical RN experience.
• CPR certification required.

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
• Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical RN experience.
• Current CPR certification.
