Registered Nurse - Home Care and Hospice

Schedule

Sunday - Thursday, 8 AM - 4 PM, with on-call rotation and holidays required.

Job Description

This is a 13-week contract position requiring a skilled Registered Nurse to provide quality care in home care and hospice settings. The nursing process will be utilized to assess patient and caregiver needs while ensuring compliance with State and Federal regulations and agency policies.

Responsibilities

• Complete and maintain OASIS documentation in accordance with agency policies and regulations.
• Assess patient and caregiver needs to provide comprehensive and individualized care.
• Develop and communicate realistic plans of care involving patients and caregivers.
• Act as Case Manager for assigned patients while managing case loads effectively.
• Implement plan of care and nursing interventions to ensure high-quality patient care.
• Stay updated on high-tech skills, including IV use, complex wound care, and infection control.
• Collaborate with LPNs to ensure adherence to the plan of care and delegate tasks appropriately.
• Evaluate the effectiveness of care and make adjustments as necessary.
• Maintain accurate documentation of nursing care provided for integration within the healthcare team.
• Follow agency protocols with McKesson processes for timely visit management.
• Educate patients and caregivers on treatment options, medication side effects, and discharge instructions.
• Identify interdisciplinary needs and communicate these effectively to relevant team members.
• Complete necessary forms timely to ensure compliance with regulations.
• Assess needs for Home Care Aide services and coordinate care plans accordingly.
• Report patient incidents to the Quality Improvement Specialist as needed.
• Coordinate safe patient discharges and transfers, ensuring all necessary paperwork is processed.
• Facilitate communication with physicians, team members, and vendors to support patient care.
• Participate in monthly peer review audits, accepting and providing constructive feedback.
• Proactively facilitate case conferences for effective communication within the team.
• Advocate for patient care issues to ensure quality and personalized care for each patient and family.

Required Experience / Certifications / Licensure

• Current Registered Nurse license valid in the state of practice.
• Experience in home care and hospice preferred.
• Knowledge of OASIS documentation and relevant regulations.
• Strong case management and communication skills.
