Job summary

Qualifications

• RN with a bachelor’s degree in nursing preferred.
• 1+ year of clinical experience as an RN, homecare experience a plus.
• Strong verbal and written communication skills.
• Proficient with EMR systems and digital tools.
• Reliable car and valid driver's license; travel to patient homes.

Responsibilities

• Assess patients with health needs and develop personalized care plans.
• Educate and empower patients and families to manage conditions.
• Collaborate with VNA clinicians and external providers for seamless care.
