VNA Health Group in New Jersey is seeking a Staff Nurse Homecare to join our mission of empowering patients to live healthier lives at home. As part of a dedicated team, you’ll assess health needs, develop personalized care plans, educate families, and deliver compassionate nursing in patients’ homes.

We value a BSN (preferred) and at least 1 year of RN experience, including homecare, strong communication, and comfort with EMR systems.

Qualifications

• RN with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical RN experience; homecare experience is a plus.
• Strong verbal and written communication skills to educate patients and families.
• Proficient with technology and EMR systems to enhance patient care.
• Must have reliable car and valid driver’s license for home visits.

Responsabilités

• Assess patients with health needs and develop personalized care plans.
• Provide high-quality, compassionate homecare nursing to patients in their homes.
• Educate patients and families on conditions and self-care.
• Collaborate with a team of clinicians and external providers for seamless care.
• Document changes in patient conditions and update care plans accordingly.
