Qualifications

• Registered Nurse (RN) with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical experience as a registered nurse (bonus points if you have experience in homecare!).
• Strong verbal and written communication skills – you’ll be guiding and educating patients and families.
• Proficient with technology – navigating EMR systems and using digital tools to enhance patient care.
• A reliable car and valid driver’s license – you’ll be traveling to patient homes, so your own transportation is key

Responsibilities

• Assess patients with health needs and develop personalized care plans.
• Educate patients and families to manage health at home.
• Coordinate care with VNA clinicians and external providers.
