VNA Health Group is seeking a Staff Nurse Homecare to deliver compassionate in-home nursing care across New Jersey. You will assess health needs, develop personalized care plans, and empower patients and families to manage conditions in the comfort of their home.

The role includes a two-week orientation; RN with BSN preferred and at least 1 year of clinical experience. Strong communication, EMR proficiency, and a valid driver’s license are essential for success.

Formación

• RN with a bachelor's degree in nursing (BSN preferred).
• At least 1 year of clinical RN experience (homecare a plus).
• Strong verbal and written communication skills.
• Proficient with EMR systems and digital tools.
• Reliable transportation with a valid driver’s license.

Responsabilidades

• Lead initial patient assessments and develop personalized care plans.
• Monitor patient health, adjust care plans as conditions change.
• Educate patients and families to support self-care at home.
• Collaborate with VNA clinicians and external providers for seamless care.
• Document changes and outcomes accurately in the EMR.
