Qualifications

• RN with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical RN experience (homecare preferred).
• Strong verbal and written communication skills.
• Proficient with EMR systems and digital tools.
• Reliable car and valid driver's license for patient home visits.

Responsibilities

• Complete a two-week orientation covering procedures, EMR, safety, and compassionate care.
• Assess patients with complex health needs and develop personalized care plans.
• Educate and guide patients and families through their healthcare journey.
• Collaborate with VNA clinicians and external providers to ensure seamless care.
• Document patient status changes and update care plans as needed.
